    Planning Your Visit     

​If this is your first time seeing Dr Farhat, please aim to arrive 10 minutes prior to your consultation time in order to complete Registration Forms â€‹

Referral Letter 

A Referral Letter is typically provided by your GP, but may be provided by another specialist. This letter explains why you have been referred to see Dr Farhat

 

- If you have been referred by the Emergency Department, please bring your Discharge Summary with you

 

- If you have any relevant investigation reports as part of your referral, please bring these along too

In order for Dr Farhat to help you best, please bring all your relevant investigation results, which include:

 

- Medical Imaging:  Xray, CT, Ultrasound, Bone Scans, and MRI images and reports.

- Blood Test Results

- Special Studies: these include things like Nerve Conduction Studies. Your referring doctor would have normally indicated what these are.

 

If you have hard copy images / reports then bring them along to your consultation. If these are not available, please take note of WHERE your images were performed so Dr Farhat can access them during your consultation.

Medicare Card

In order for us to process your medicare refund we require your medicare card at the time of consultation

Private Health Insurance Details

Dr Farhat works with all health insurance providers.

 

If you have private Health Insurance Cover, please bring along your relevant Insurance Card or Insurance Details. 

 

Where surgery is required, Dr Farhat and his team will be able to discuss the different options available to you.

Workers Compensation / WorkCover Claims

For work-related injuries where a claim has been made

Please bring in all Claim Details. 

 

 These include:

- Claim Number

- Employer details

- Case manager name and contact details

CTP / Insured Road Accidents 

For all accidents that have occurred on a public road where a CTP (Compulsory Third Party) claim has been made, please provide your claim details including:

- Claim number

- Insurer
